After you've paid, you will be taken to a "license page" where you will see a license for jp FirstRate.

Install the zip file as a mambot (Installer->mambot) and publish the mambot.
Unpublish the mambot called "MOS Rating."

To make sure you have the rating enabled, go to Site->global configuration and choose YES next to "Item Rating/Voting" on the "content" tab.
